VBO Hockey – Photo & Video Release / Consent Form

I, [Participant's Name], hereby give my consent to VBO Hockey, its coaches, staff, representatives, and affiliated personnel to capture video footage and photographs of me during my participation in VBO Hockey programs, events, and related activities.

I understand that these images and recordings may be used by VBO Hockey for advertising, marketing, and promotional purposes, including but not limited to use in print materials, websites, social media platforms, and other promotional content.

I acknowledge and agree that:

• I waive any right to inspect or approve the final materials where my likeness appears.

• I waive any claim to royalties, compensation, or other benefits from the use of these photos or videos.

• I release and hold harmless VBO Hockey, its staff, coaches, and representatives from any and all claims related to the use of my image, voice, or likeness — including claims for defamation, invasion of privacy, or violation of moral or publicity rights.

I affirm that I am at least 18 years of age and legally competent to provide this consent. If I am under 18, I confirm that my parent or legal guardian has given consent on my behalf.

By participating in any VBO Hockey program — including but not limited to private training, team sessions, clinics, camps, hockey schools, or off-ice activities — I acknowledge and accept the terms of this release.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, PHOTO & VIDEO RELEASE AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT VOLUNTARILY, WITHOUT COERCION OR PROMISE OF COMPENSATION. I INTEND FOR MY AGREEMENT TO SERVE AS A COMPLETE AND UNCONDITIONAL RELEASE OF LIABILITY TO THE FULLEST EXTENT PERMITTED BY LAW.
